ABSTRACT

A new series of cold brand reactive dyes (D1 to D12) were synthesized by the coupling of diazotized 4-Amino-N-(5- methylisoxazol-3-yl)-benzenesulfonamide with various cyanurated coupling components in good yield. These reactive dyes were confirmed by 1H NMR analysis and FTIR. The synthesized dyes have been applied to cotton, silk, and wool fibres. They exhibited a variety of color shades with good dept and informality on the fibres. % Exhaustion and % fixation of dyes were determined by using Glauber salt as a fixing agent at various temperature conditions.

ABSTRACT

Background: Dapsone treatment may reduce HbA1c levels in patients with diabetes. Aims: To assess the prevalence and characteristics of dapsone associated reduction of HbA1c in patients with Hansen’s disease. Methods: A retrospective data review of outpatient and inpatient charts of consecutive patients with Hansen’s disease and type 2 diabetes mellitus was conducted over two years from January 2014 to January 2016 at the Department of Dermatology, CMC Vellore, India. Results: Of the 245 patients with a confirmed diagnosis of Hansen’s disease who were on oral dapsone 100 mg/day as part of their treatment regimen, 49 patients had diabetes and were eligible for the study as per predetermined inclusion criteria. Of these, 35 subjects (71%) had an HbA1c discordantly lower than the corresponding mean plasma glucose levels. Patients with discordant HbA1c levels were more likely to be male and to have a higher RBC mean corpuscular volume (MCV). A greater reduction in HbA1c levels was seen during the initial 3 months of therapy of dapsone treatment. Limitations: The small sample size and retrospective design were limitations of this study. Also, we did not analyze the role of methemoglobinemia or the utility of alternative measures of glycemic control in these patients. Conclusion: We describe a high prevalence of dapsone associated inappropriate HbA1c lowering in type 2 diabetes mellitus patients. This may have serious implications for the management of diabetes in patients on therapy with dapsone.

ABSTRACT

Background: The altered spectrum of diseases in geriatric age group and atypical manifestations of these conditions make geriatric health care truly unique. The challenge ahead for health care in the coming years is to ensure the quality of life to a large group of the elderly population. In spite of the recognition that the elderly has specific medical conditions, a dedicated health care policy to improve geriatric health care is yet to be developed India. Objectives: To study the disease pattern, hospital stay and outcome among the geriatric patients based on age, gender and other variables. Materials and methods: An observational prospective study done after permission of human research ethics committee. All patients aged 60 years and over admitted to the wards were included into the study. Detailed information about current admission and other history was noted. Thorough clinical evaluation was done by physical examination and investigations and noted in the case record form. Observations: Total 100 participants included in the study. They were divided into two groups 60-64 years and ? 65 years. Nearly 90% patients had more than one aliments or diagnosis. Non-communicable diseases like Cardiovascular disease (48%) was the commonest reason for admission, followed by Respiratory disease (42%), renal disease (40%), gastrointestinal disease, neurological disease and endocrine disease in descending order of frequency. Conclusion: Non-communicable diseases are common cause of morbidity and mortality with infection are adding the morbidity as well as mortality among the elder population.

ABSTRACT

Background: The surgical and procedural specialties are continually evolving their methods to include more complex and technically difficult cases. These cases can be longer and incorporate multiple teams in a different model of operating room synergy. Patients are frequently older, with comorbidities adding to the complexity of these cases. Recording of this environment has become more feasible recently with advancement in video and audio capture systems often used in the simulation realm. Aims: We began using live capture to record a new procedure shortly after starting these cases in our institution. This has provided continued assessment and evaluation of live procedures. The goal of this was to improve human factors and situational challenges by review and debriefing. Setting and Design: B‑Line Medical’s LiveCapture video system was used to record successive transcatheter aortic valve replacement (TAVR) procedures in our cardiac catheterization/laboratory. An illustrative case is used to discuss analysis and debriefing of the case using this system. Results and Conclusions: An illustrative case is presented that resulted in long‑term changes to our approach of these cases. The video capture documented rare events during one of our TAVR procedures. Analysis and debriefing led to definitive changes in our practice. While there are hurdles to the use of this technology in every institution, the role for the ongoing use of video capture, analysis, and debriefing may play an important role in the future of patient safety and human factors analysis in the operating environment.

ABSTRACT

Extracorporeal membrane oxygenation (ECMO) for severe acute respiratory failure was proposed more than 40 years ago. Despite the publication of the ARDSNet study and adoption of lung protective ventilation, the mortality for acute respiratory failure due to acute respiratory distress syndrome has continued to remain high. This technology has evolved over the past couple of decades and has been noted to be safe and successful, especially during the worldwide H1N1 influenza pandemic with good survival rates. The primary indications for ECMO in acute respiratory failure include severe refractory hypoxemic and hypercarbic respiratory failure in spite of maximum lung protective ventilatory support. Various triage criteria have been described and published. Contraindications exist when application of ECMO may be futile or technically impossible. Knowledge and appreciation of the circuit, cannulae, and the physiology of gas exchange with ECMO are necessary to ensure lung rest, efficiency of oxygenation, and ventilation as well as troubleshooting problems. Anticoagulation is a major concern with ECMO, and the evidence is evolving with respect to diagnostic testing and use of anticoagulants. Clinical management of the patient includes comprehensive critical care addressing sedation and neurologic issues, ensuring lung recruitment, diuresis, early enteral nutrition, treatment and surveillance of infections, and multisystem organ support. Newer technology that delinks oxygenation and ventilation by extracorporeal carbon dioxide removal may lead to ultra‑lung protective ventilation, avoidance of endotracheal intubation in some situations, and ambulatory therapies as a bridge to lung transplantation. Risks, complications, and long‑term outcomes and resources need to be considered and weighed in before widespread application. Ethical challenges are a reality and a multidisciplinary approach that should be adopted for every case in consideration.

ABSTRACT

Objective: The aim of this study is to determine the efficacy of methotrexate treatment for ectopic pregnancies in our settings, which will in-turn help us reduce maternal mortality and morbidity. Material and methods: This was a retrospective study of 40 cases of unruptured ectopic pregnancy treated with methotrexate in civil hospital, Ahmedabad. Clinical presentation, treatment, progress, outcome, side effects and future fertility follow-up were analyzed using database from May 2008 to June 2012. Results: The success rate of methotrexate therapy in our study was 85% (n = 35) and 15% (n = 06) required surgical intervention compared to reported success rate of 67-100% published in various studies. The mean average time of resolution of ectopic pregnancy was 32 days for a single dose and 58 days for repeat second doses. Successful fertility outcome with intrauterine pregnancy was observed in three patients. Conclusion: Methotrexate treatment of ectopic pregnancies is safe and effective with no major side effects. Intramuscular methotrexate has the advantage of tubal conservation and saves patients from surgical intervention. Our study showed single dose methotrexate to be an effective treatment option for selected patients with unruptured tubal ectopic pregnancy.

Malaria is one of the foremost public health problems in India. Objectives: To study the clinical spectrum, complications & treatment outcome in smear positive hospitalized children having malaria. Method: this prospective observational study enrolled 190 smear positive malaria patients admitted in pediatric wards of general hospital during 6 months. History taken & examination done. All patients were investigated & treated according to WHO guidelines & followed twice daily till their hospital stay. Results: 88% of patients were having P. vivax malaria. Males were 2.1 times more commonly affected. Age group most commonly affected was 5 to 10 years. Highest no of cases were reported in month of August. All patients were having history of fever. Thrombocytopenia was frequently associated (87%). Cerebral malaria was the commonest complication. Almost all patients with uncomplicated vivax malaria responded to Chloroquine. CFR was 1.05%.

ABSTRACT

Solid-state fermentation of wheat straw was carried out by a native white rot basidiomycete Daedaleopsis flavida strain 5A. Extract prepared from the 12-day decayed wheat straw contained extracellular ligninolytic enzymes like manganese peroxidase (MnP), manganese-independent peroxidase (MIP), lignin peroxidase (LiP) and laccase along with straw-degraded products and pigments. Sephacryl S-200 size exclusion chromatography in 16/100 column was used for the separation of these ligninolytic enzymes and strawdegraded products and pigments. Recovery of pigment-free ligninolytic enzyme activities as protein was 40% of the total proteins loaded and specific LiP activity increased 34 fold after size exclusion chromatography. Thus accurate estimation of LiP by veratryl alcohol oxidation assay was possible only after the removal of interfering pigments. The reproducibility of size exclusion chromatography is adjudged satisfactory from the consistent results obtained after seven repetitive uses of matrices.

Biofertilizers are an alternative to mineral fertilizers for increasing soil productivity and plant growth in sustainable agriculture. The objective of this study wheat plant growth promoting by non symbiotic (Azotobacter spp.)Strains as biofertilizer on soil properties and seedling growth of wheat in soil, and the application treatments included the control (without bacteria inoculation), non symbiotic organism isolated saline desert soil in which studied soil all properties. Checked nitrogen fixing activity, IAA producing activity and studied activity against pesticide of all isolated and record results.

Kinetic properties of rat liver acid phosphatase were evaluated using the conventional synthetic substrates sodium beta glycerophosphate (betaGP) and p-nitrophenyl phosphate (PNPP) and physiologically occurring phosphate esters of carbohydrates, vitamins and nucleotides. The extent of hydrolysis varied depending on the substrates; phosphate esters of vitamins and carbohydrates were in general poor substrates. Kinetic analysis revealed the presence of two components of the enzyme for all the substrates. Component I had low Km and low Vmas. Opposite was true for component II. The Km values were generally high for betaGP, PNPP and adenosine diphosphate (ADP). Amongst the nucleotides substrates AMP showed high affinity i.e. low Km. The increase in enzyme activity in general at high substrate concentration seems to be due to substrate binding and positive cooperativity. AMP which showed highest affinity was inhibitory at high concentration beyond 1 mM. The results suggest that in situ the nucleotides may be the preferred substrates for acid phosphatase.
